Seven Social Care is looking for a qualified Social Work Team Manager to fill an exclusive opportunity specialising in the wellbeing of Children as part of the Leaving Care Service in Milton Keynes.

The role:

You will be responsible for the successful transition of all children in care through to adulthood and support those young people up until the age of 25 years.

Key Responsibilities:

You ensure all young people have an up to date Pathway Plan that is SMART and recognises the young person's needs.
You will review performance data to ensure KPI's are on track and identify where young people might need additional support.
You will manage a Team of 14 including a Deputy Team Manager, Senior Practitioners, Social Worker and Personal Advisors.
You will attend and chair key meetings in regards to Young People, including, strategy meetings, housing panel, Education, Employment and Training panel.
You will support colleagues across Corporate parenting services and contribute to wider service development
You will ensure have regular supervision and through supervision ensure quality of work and progress of care plans.
Applicant Requirements:

Qualifications: Social Work Degree approved by the Health and Care Professions Council (HCPC).
Experience: Experience as a Team Manager with Leaving Care.
Compliance: Up to date HCPC registration and DBS.
Travel: Clean UK driving license with the ability to commute to and from work independently.
Hours: 37.5 hours p/w.
