A challenging, yet rewarding role supporting young people in their journey to adulthood. You will be based in a home which is the residence of 3 young people, providing them with emotional and practical support to promote their well-being and independence.
You will work full day shifts and also 24 hour shifts where you will "sleep in". Being on hand to keep the young people on track with their behaviour, education, work, finances, health and relationships.
This position involves lone working with the support of the Team Manager who will visit and support the service and the support team daily.
Your key duties and responsibilities as a Support Worker will include:
* Being responsible for ensuring that the needs of 3 young people are met, guiding them towards the achievement of positive independent living skills.
* Providing advice to young people in all areas of daily life, including budgeting, health, careers, education, relationships, culture, identity, accommodation maintenance and safety.
* Liaising with the young people’s Social Workers and working in partnership with external agencies to provide the best possible holistic care.
* Formulating, implementing and reviewing care plans.
* Writing weekly reports, keeping young people’s files up to date and informing management of any concerns that you have for any of the young people in regards to their welfare or social and family problems.
* Supporting the young people in meetings, making sure that their views are heard and taken into consideration along with transporting them to meetings and appointments.
* Ensuring that the accommodation is kept in good condition and that the young people’s utility bills are correctly organised and paid on time.
Due the lone working that this position requires it is essential for candidates to have previous experience of working with children/young people who have complex needs and challenging behaviour.
This experience is vital as the successful candidate will:
* Have an excellent working knowledge of safeguarding for young people in care
* Be able to quickly build a rapport with often hard to reach individuals
* Show that they can confidently assess and respond to high-risk situations
Candidates are also required to be car drivers with the use of their own car. Your mileage will be paid
