This is a brand-new opportunity for a Social Worker to join the Hospital Reablement Team. This a Locum Social Work position based in Milton Keynes. The Social Worker will be working with cases caused from COVID – 19 and general Hospital D2A. The Social Worker will also be working within a busy environment with expectation to work to KPI's.

Your responsibilities as a Social Work

Implement care plans and care packages
Carry out Care Act 2014 assessments
Carry out MCA assessments
Undertake Section 42 enquires
Carry out the best level of care and services for the benefit of the service user
Deal with Discharge to Assess
Carry out CHC checklist
Work to KPI's

Requirements of a Social Worker

Current registration with Social Work England
2 years post qualifying UK experience
Qualified Social Worker BA/DipSW/CQSW or equivalent
Must be double Vaccinated for this post
