A large healthcare organisation is looking to clinical professional, please note to be considered for this role you must have a current registration to the appropriate body (i.e. NMC, HPCP, Social Work). The role will be focused on boosting out of hospital care.
Main Responsibilities:
The Ageing Well Programme is an element of the NHS Long Term Plan which focuses on boosting out of hospital care, keeping people well and at home, and improving outcomes for people. Its aim is to ensure people get the right help early so they don’t need to go to hospital in the first place;
The Division of Integrated Elderly and Community Care has two pilots commencing in October to deliver new and enhanced models of care which adhere to the spirit of Ageing Well. The pilots are new patient pathways with dedicated investment and will guide and inform future pathways for patients to keep people out of hospital and in their homes, as well as facilitating safe, swift discharges from A&E and hospital;
This Clinical project Manager post is for a senior leader for a 6 month contract, working with the clinical team and Service Leads, in the delivery of the pilot which specifically focuses on enhancing the community therapy service (RRIC) to deliver an urgent 2 hour community response to patients at home to avoid unnecessary hospital attendance and admissions and to also support patient’s home from A&E;
The post holder will be responsible for the coordination and implementation of the project plan which encompasses the planning, monitoring and evaluation of the pilot, problem-solving, working with stakeholders and clinical practitioners ensuring that pilot and workgroup deliverables are met within the timelines defined.

This post requires a person who has advanced clinical experience working in the community and ideally clinical experience in A&E. They will need to have enthusiasm, vision, project experience and leadership to drive the pilot forward;
They will need excellent people skills to support, influence and lead on this new patient pathway due to the overlaps and complexity it brings working across a range of healthcare professionals and service managers in the acute and community teams in BHT, as well as multiagency working with Social Care;
