RMN or Social Worker – Ayelsbury – HOME WORKING

What they offer:

£27 / hour – also depending on experience
ASAP start – On-going
Monday -Friday
9am – 5pm

About the role:

Predominantly home working, however visits to patients are required at times for face to face support or for medication needs.
Duty worker is based at the Centre – team office, but frequency of this is for one week in eight approximately.
The candidate will be holding a caseload of approximately 28 cases, dependent on complexity and will be supporting patients in the community with a variety of mental health and social care needs. 
Requirements:

Experience of care coordination work is essential
Car driver
 
Responsibilities: 

To deliver a high-quality nursing service to Older People with mental health problems, in a multi – professional framework.

You will be providing high quality services to older people with mental ill health and dementia in the community.
