Service Care Solutions have a vacancy available for a Band 6 Physiotherapist for a role based in Bletchley, Milton Keynes.

The successful candidate will be required to work in community admission avoidance and supported discharge team in Bletchley. The role will include completing multifactorial assessments to identify goals and treatment plans, providing intervention such as issuing equipment and rehabilitation plans to improve functional independence. The role includes working closely with the immediate MDT consisting of rehabilitation assistants, OTs, nurses and medicines management technician.

Experience of working with older people is essential. Car driver essential as this is a community post and you will be working in patients own homes.

Previous experience in the NHS is essential. Please do not apply if you have not worked within the NHS previously.

The role is full time, 8:30 – 5pm Monday to Friday.
