Healthcare Assistant band 4 – needed in Milton Keynes

Between £15 – £17 /hour – depending on experience

About the role:

An exciting opportunity has become available in our service to work with the Rapid Engagement and Support Team (REST), supporting discharge coordination within the Centre during the winter period.
Aims of the Team: Patient safety, support and suicide prevention, Reduction in MH bed pressures and access to MH beds, Reduction in A&E pressures.

Responsibilities:

You will be responsible for the provision of recovery orientated, practical support to patients discharged to help re-orientate within the home environment and navigate local community support networks, including mutual aid.
 
Requirements:

Previous experience as a healthcare assistant or support worker
