We are looking for a Complex Care Nurse (case load) Manager to join a well-established and progressive provider of complex care packages within the community. This community role will be based in the Buckinghamshire area where the company has secured further packages during an exciting growth phase.

The successful Complex Care Nurse (case load) Manager will be responsible for providing face to face support to Adults or Paediatrics, whilst creating personalised care plans for each service user. You will work closely with a core team of carers; training and supporting them with their clinical skills and activities.

Criteria for a Complex Care Nurse (case load) Manager:

RGN / RSCN with a minimum of 6 months in the UK and an active NMC PIN
Supportive management style that can bring out the best in others
Up to date experience of tracheostomy care and ventilators within the last 2 years
Advanced communication skills verbally and written to liaise effectively with patients, families and the MDT
Suitable for Band 5 /Band 6 Nurses with evidence of effective leadership and management of HCAs
Full UK Driving Licence desirable but not essential

Key responsibilities for a Complex Care Nurse (case load) Manager:

Design and deliver a tailored care plan specific to an individual client's needs
Effectively manage the MDT through strong communication and teamwork
To provide ongoing monitoring, support and assessment of client's physical and physiological needs within your caseload
To deliver nursing interventions including the management of medicine delivery
Build and maintain sophisticated relationships with your client's family
Working closely with community teams, CHC nurses and CCG commissioners to establish and maintain bespoke packages of care

Up to £45,000 per annum DOE
Monday to Friday hours (9-5)
