TLTP Medical are looking for a Band 7 Frailty Practitioner (PT/OT) based in Buckinghamshire. The role is due to start ASAP for a minimum of 6 months.    
       
The suitable applicant will need to have recent NHS experience of working in a rapid response/intermediate care setting.

As part of this pilot, they are looking for a frailty practitioner (band 7) who will work in the emergency department as part of the font door MDT team called REACT (nurses, PT, OT and support workers) to enable patients who can be turned around at the front door to be discharged home and have rapid support from RRIC within 2 hours if this is required to enable them to return home safely.

Must also have experience of working in the community and ideally clinical experience in A&E

The shifts will be across 7 days and a mix between 8-4pm and 12-8pm
       
Pay: £28-£30/h (Umbrella)
