Eden Brown Synergy have an exciting opportunity for a Qualified Social Worker to join the Chiltern CMHT based in High Wycombe, Buckinghamshire.

The post is for an initial three months, although could be extended for the right candidate, and is paid at NHS Band 6 rates. This is a full-time role (37.5 hours) Monday-Friday 09:00-17:00. The team are currently operating a WFH rota, providing opportunity for clinicians to complete some aspects of work from home. On these WFH days clinicians may still be required to conduct face to face visits, according to service needs. Any specific requirements can be discussed with managers.

The post-holder will be undertaking full care act assessments in line with current legislation to support hospital discharges within the community, determining peoples long term needs and supporting carers.

Key experience required:

Degree or diploma in Social Work and SWE registration
Previous experience working within a CMHT
Full UK drivers license and access to a vehicle
Clear experience and operational knowledge of working in a busy and pressurised front-line team
Excellent communication skills (written and verbal) one to one and with client groups
Complex case management; working with children and families as well as adults
Experience working in the community
Good collaborative skills, maintaining strong relationships with partners
Safeguarding and continued Healthcare
Sound understanding of current Social Care legislation including Care Act, Mental Capacity Act, Mental Health Act etc
Excellent organisational and prioritisation skills
Able to work independently to strict deadlines under pressure
To demonstrate person-centred working
Ability to utilise ICT at a high level and adapt to new ICT systems
