Care Coordinator

Location: Beaconsfield, Buckinghamshire

Universal Care is a highly reputable independent care company based in Beaconsfield. We currently have an exciting opportunity for an experienced Care Coordinator to take on this exciting and important role in our company. Our Domiciliary Home Care services offers high quality bespoke care to our clients, mostly private and we are now looking for someone to ensure that our standards are maintained.

Experience Required:

You will be experienced within the social care sector, ideally come from a background with a history of performing within a similar role, have experience of scheduling or logistical experience. You will be an excellent communicator, intelligent, well organised with excellent attention to detail.

Able to think methodically and strategically.

Your New Role:

* As a Care Coordinator you will be responsible for a designated area, based in Beaconsfield but must be mobile to visit our clients and carers.

* Ensure all client care calls are allocated to a carer through efficient rostering.

* To maintain and update client and carer databases with relevant information.

* You can drive and have access to your own transport and preferably live within a 15-mile radius of our office.

* Your working week will be 37.5 hours Monday to Friday, but you will be required to participate in out of hours on call rota at a maximum frequency of one in four weekends plus weekday evenings.

* To participate in client care planning and the supervision and support of carers.

* Build positive relationships with clients, families and professional partners.
