Eden Brown Synergy have an exciting opportunity for a Qualified Social Worker to join the Aylesbury and Vale Community Mental Health Team in Buckinghamshire as Social Care Lead.
This is a full-time position paid at NHS Band 7. The hours of work will be Monday-Friday 09:00-17:00 with some working from home to be agreed with the candidate. We are looking for someone to start immediately on an ongoing basis (reviewed monthly).
The role has been described as follows:
Social Care Lead – managing a team of six social workers within an integrated CMHT – responsible for all social care responsibilities (Care Act assessments, care and support planning, reviews) and safeguarding that are delegated under the section 75 agreement between the health trust and the local authority.
Key experience required:
Experiencing managing and supervising a social work team, preferably in a mental health service.
Strong leadership skills
Excellent communication skills (written and verbal) one to one and with client groups
Complex case management; working with children and families as well as adults
Good collaborative skills, maintaining strong relationships with partners
Safeguarding and continued Healthcare
Sound understanding of current Social Care legislation including Care Act, Mental Capacity Act, Mental Health Act etc
Excellent organisational and prioritisation skills
Able to work independently to strict deadlines under pressure
To demonstrate person-centred working
Ability to utilise ICT at a high level and adapt to new ICT systems
Car driver essential
Other essential requirements:
DIPSW Qualification and Registration under Social Work England
Evidence of Continuing Professional Development
To hold an advanced professional / clinical qualification
Desire to train as an AMHP
Experience and knowledge of completing detailed and comprehensive assessments of complex health and social care needs through the practices of Care Coordination / Care Management and the application of funded social care, placing the service user at the centre of practice
