Eden Brown Synergy have an exciting opportunity for a Qualified Social Worker or Occupational Therapist with community experience to join the North Bucks Older Adults CMHT based in Aylesbury, Buckinghamshire.

The post is for an initial six months, although could be extended for the right candidate, and is paid at NHS Band 6 rate. This is a full-time role (37.5 hours) within a service that runs Monday-Sunday 09:00-17:00 and occasionally 12:00-20:00 Monday-Friday. There will be scope for some remote working.

The post-holder will be undertaking full care act assessments in line with current legislation to support hospital discharges within the community, determining peoples long term needs and supporting carers.

Key experience required:

Degree/diploma in Social Work and SWE registration
OR Degree/diploma in Occupational Therapy and HCPC registration
Full UK drivers license and access to a vehicle
Clear experience and operational knowledge of working in a busy and pressurised front-line team
Excellent communication skills (written and verbal) one to one and with client groups
Complex case management; working with children and families as well as adults
Experience working in the community
Good collaborative skills, maintaining strong relationships with partners
Safeguarding and continued Healthcare
Sound understanding of current Social Care legislation including Care Act, Mental Capacity Act, Mental Health Act etc
Excellent organisational and prioritisation skills
Able to work independently to strict deadlines under pressure
To demonstrate person-centred working
Ability to utilise ICT at a high level and adapt to new ICT systems
