Qualified Social Worker required for a Community Learning Disabilities Team in Milton Keynes £30ph

Duties include:

Provide a person-centred approach to adults with learning disabilities and their family carers.
Work within a multi-disciplinary health and social care service.
Conduct assessments and reviews that are proportionate to need.
Identify risks to the individual and respond appropriately, ensuring any safeguarding issues are addressed.
Assess the needs of vulnerable adults and their carers, and to support them to arrange services to meet the needs and outcomes identified.
Undertaking face to face visits occasionally

Additional Information on the position:

£30 per hour Umbrella
Remote working is in place but required to do visits
Interviews arranged via Microsoft Teams
Full time position
Start dates ASAP
3 months on-going contractBelow is essential if you are interested in applying for the role:

Qualified Social Work Degree – Hold full HCPC registration – DBS check required/Update Service – Experience working with Adults and Older people
