Athona is currently recruiting for an ongoing locum Band 6 Continuing Care Nurse Assessor.

The post holder will be working remotely as part of Continuing Healthcare Team.

The role will require carrying out new assessments and reviews of continuing care caseload, present cases at panel, formulate/update care prescriptions, case management of a caseload, liaison with other professionals and care providers.

The successful candidate will be experienced in Continuing care; knowledge of Continuing Healthcare (adult framework) is required. Will be remote working so needs to be competent/confident carrying out assessments/reviews etc over Teams. No expectation for travel except on the first day to pick up equipment from the office.
